When filling out the application, please complete the following:

  1. Print out the application in your format of preference.
  2. Please fill in the application COMPLETELY. Including any and all financial information, and the information of those that will be habitating the property with you.
  3. Submit a check to CR Medina in the amount of $25.00 for the non-refundable processing fee.

  5. If you would like, you may fax the application to (216) 696-5499 Attention: Tracy, however, processing will not commence without the $25.00 payment which may be mailed to the address above.
